TRY OUT REGISTRATION: Must be filled out before a tryout takes place. This form allows a player to try out for one of our teams or an entire team to be considered to become an affiliate of the AZ CLASH Organization. If you would like to be considered, please complete the following form below. If you missed one of our main tryouts, we can also set up an individual workout during one of our weekly practices and/or TAKE A PRO, COLLEGE OR ANY AZ CLASH COACH/AFFILIATE/SCOUT'S RECOMMENDATION TO BE ACCEPTED INTO OUR PROGRAM. |

TEAM/COACH/DIRECTOR-Affiliate Agreement, for teams and coaches only! This form allows a AZ CLASH Director, Coach or Team Affiliate to register, provide contact and profile information for the website, and SSN for payment to the Contractor after work is completed.(Contractor will receive a 1099 from our management Company C.S.M.) if paid by us. |

2025 AZ CLASH/AZ Premier Cornhole Tournament at Stetson Hills : Deem Hills Park, Phoenix AZ, Saturday April 12th at 10:00am until Done This is a fundraiser for the AZ CLASH Baseball Teams. A payment of $50 per CornHole team must be submitted by April 10th to lead fundraising person on your players team.
