I acknowledge and agree that all Jersey All Sports softball leagues require that all catchers to wear a protective mask during league play. I understand the risks involved and the potential injury if a member of my team decides to not wear one while participating at the catcher's position. I, myself and teammates do hereby release and forever discharge Jersey All Sports, the league administrator, and umpires of and from all liabilities, claims occurring from an injury which may occur while not wearing a protective mask.

I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.
